You can apply for your fire permit, consultation, or construction permit by either MANUAL submittal or ELECTRONIC submittal, by completing the following steps:

Steps for MANUAL submittal:

1. Download the Permit Application Form above
2. Print form and fill out manually and sign.
3. Mail form with a check for required fees to:

Steps for ELECTRONIC submittal:

1. Download the Permit Application Form above
2. Open the Permit Application Form and fill out all applicable fields.
3. Sign form either electronically (if available to you), or print, sign manually, then scan and save the signed form.
4. To send permit application, scroll down and fill out the email form below (where indicated upload the signed Permit Application Form), then click ‘Submit’ at the bottom of the page.
5. To pay required fees, click here: Fee payment via PayPal. NOTE: you will be redirected to the FMFPD PayPal donation site.
   1. At the site, enter the required fee amount.
   2. Select the option ‘Add $<xx.xx> USD to cover the fees’.
   3. When paying, add a note to advise that this payment is a ‘fee for service’ (and any other application information, such as owner or project name).
   4. Submit payment.
